2024-2025 STUDENT SPORTS PHYSICALS/ PAPERWORK UPDATE

Dear Parent/ Guardian of a Union City Area School District Student,

All Union City Middle or High School athletes that are interested in participating in a FALL, WINTER and or SPRING sport in the 2024-2025 school year that did not get a physical already are highly encouraged to get a sports physical at school on Wednesday, June 19th. (Cost: $10) Physicals will start at 1:00 PM.

The PIAA requires ALL sport athletes in grades 7th through 12th to have a physical completed and submitted before the first day of mandatory practice. 

PIAA sports physicals begin and expire with the school year (May1st- April 30th). Any physical dated before May 1st, 2024 will not be accepted for the upcoming school year.

ALL PHYSICAL PAPERWORK WILL BE DONE ONLINE  at     ucsd2.atsusers.com

ALL PHYSICAL PAPERWORK MUST BE COMPLETED online PRIOR TO ATTENDING THE PHYSICAL DAY.

If online paperwork is not completed or $10 payment is not present your student athlete will not be able to receive a physical and unfortunately will be turned away. These students will need to get a physical with your primary care physician. Checks should be made payable to: Union City Area School District 

Every year you will need to complete the online PIAA sections 1-5 physical paperwork online.

No paper copies will be accepted, unless you go to your primary care physician and they fill out the PIAA section 6.
